FWIW, This is a common problem with the "patch" program and patches that 
contain new files.  If you apply the patch, it creates the new file, but 
if you reverse the patch, it doesn't delete it.  Then, if you re-apply 
the patch, it silently appends the contents of the "new" file to the end 
of the existing file.

I'm sort of surprised that this hasn't been considered a bug and fixed 
quite some time ago, but apparently it hasn't.
