[PATCH v1 0/2] Strengthen exception guarantee for mdspan.

Checking our implementation of <mdspan> against the testsuite of libc++,
revealed differences in noexcept guarantees [1].

Libc++ performed two types of strengthening. These patches contain
simiar or same strengthening.
