[patch] Fix generation of libstdc++ API docs with Doxygen

This fixes some non-UTF-8 accented characters and updates the
run_doxygen script to work with Doxygen 1.8.x, and also updates the
doxyfile to the latest format.

Committed to trunk. I'll make the run_doxygen and tr2/bool_set changes
on the active branches too, so I can generate the docs for the next
releases more easily.
