[v3] docbook conversion of existing pb_ds docs

As threatened, here is a version of the policy-based data structures
documentation, converted from html/xhtml into the unified documentation
format (docbook 5). 

I believe that means that libstdc++ only has one doc format now. So,
took four years, but that's cool.... 

While doing this, I noticed a couple of things. 

1) current bibliography markup is wrong, will fix
2) some of the imagedata markup is wrong, will fix
3) some kind of css styling would be nice for the manual
4) some existing doxygen markup, outside of pb_ds, is kill pdf
generation. I'd filed a bug about this upstream with doxygen, but it
was closed due to extreme vagueness. I suppose the solution is to edit
doc/doxygen/user.cfg.in to remove bits of the FILELIST until pdf
generation runs to completion, and then try to get it to a single file,
and then to a specific markup instance.

I've verified that html/pdf/xml forms are coherent. And tried to do
some editing.
