[v3] Debug-mode fixes

Hi,

we didn't run the testsuite in debug-mode much lately, at this shows...
The below fixes all the recent regressions besides the failures in the -
annoying as usual - pb_ds area: something seems seriously broken or
triggering undefined behavior, whatever (for compile-time issues we
should really update the code to use the new ext/throw_allocator.h
facilities, I commented out for now the old uses of
__gnu_cxx::throw_allocator).

Tested x86_64-linux, committed to mainline.
