[v3] unified TOC for docs, phase 1

yo people. I'm back.

Here's the first pass at cleaning up the libstdc++ docs. I have a
master plan for this, which I will post in another thread.

This patch cleans up some of the dead files, removes some blatant lies,
and structures the existing libstdc++ content into a single table of
contents.

I did this and the doxygen parts of this a week ago, and sent the other
maintainers a heads up and RFC, but then before I could
check in, various hardware issues derailed me. So, I'm checking in
what I had before I went down.
