[v3] qualify namespace std items

This is the first part of the solaris include work. This is the only
part of this work that is ready to be checked in.

Basically, when "C" includes like cmath are correct, and in namespace
std exclusively, some of the current libstdc++ code is wrong, and
needs to be fully qualified.  This patch fixes all known build
failures.

tested x86/linux
