[v3] mt_allocator performance improvements

This patch was waiting on assignment, so some of these dates on the
performance testresults may seem a bit old. However, it is expected the
performance results are are similar on current sources.

Attached are the 'make check-performance' results on three configs:

single P4
dual opteron 846
dual PPC970FX 

Some of the tests (insert-thread, producer-consumer) show a nice speed bump.

This is in on the libstdcxx_so_7-branch.
