Tuple progress - help with istream / ostream

LWG just killed tuple io. So, don't worry about it.

There have been tuple changes, but my suggestion is to
not wait for everything to settle down absolutely, and
just submit what you have now, and fix up stuff to the
final TR1 wording when it is indeed final.
