This is part one of two.
This is a refined organization of locale and io code for static linkage.
What's being attempted is to keep the io and locale initialization code
separate from the non-initialization, functional code.

In particular, the goal is to eventually have locale::classic callable
without dragging in the locale instantiations and named locale bits. I'm
still working on that.
Merging this into mainline is getting to be a pain, and so I'd like to
check this in as-is. This shaves another 600k off of string::assign's
test cases, so isn't worthless even without the finished locale
separation.
