It currently does that in the multithreaded configuration, primarily to
intercept thread creation.  That's probably not the right thing to do once
the invocation API is in place.  I've already received a complaint that it's
not robust against threads started before the DLL is linked.  There's no
obvious way to fix it for C/C++ clients, but for Java clients we potentially
have other ways to deal with thread creation.
