Patch: cleaning up THANKS

I've semi-unified the THANKS and thanks.html files.  Eventually they
should really come from the same place.  For now they will require
double entry.  This is a risky file to maintain for fear of leaving
people out, but I believe it is important.  Please help maintain it.
