That depends on whether the CFG is available.  When it is, the
RTL emit machinery should set the BLOCK_FOR_INSN automatically.  
No pass can legally insert new insns in the stream and not set
BLOCK_FOR_INSN.  When it happens for you, you've found a bug.

When the CFG is not available, BLOCK_FOR_INSN should be NULL
for each insn.
