The FSF has had too many problems with people filling out the wrong 
forms, of filing them out incorrectly, wasting the time of the FSF 
copyright clerk.  So FSF policy forbids us from putting the forms 
anyplace publicly accessible.  Instead the forms should be requested 
from the FSF copyright clerk.  We can help with a little prequalifying, 
helping decide who should ask for forms and which forms they should ask for.
