Copyright Assignment Forms

I would like to begin the (potentially long) process of getting a
copyright assignment on file with the FSF.  To that end, I would
appreciate it if someone would let me know exactly what I need to do
to get the appropriate forms, and what I should do with said forms
once I get them.
