macros in .md files

Well, we can always use our own traditional preprocessor for that.
Except in case of a Canadian cross, in which case we already depend on
the actual compiler being GCC.

But I also dislike the idea of running an `md' file through a C
preprocessor.  I'd rather have `reg' to accept not only numbers, but
also names, that it would assume to be preprocessor macros.
