Where does the "__Fv" come from that gets added to all my names?

A C++ function called "foo" gets mangled into "foo__Fv".
I've noticed that "__Fv" gets added to the end of some of my names,
but not others.

Where does this appear in the source code?  What determines when
this mangling is done?
