[PATCH, Fortran] fix EQUIVALENCE in MODULE issues

Hi Steve,

The patch did last from gcc-4.2, apparently without anybody detecting
a problem :-)

Anyway, the patch looks good to me too.  Thanks for getting to it so fast, FX.
