[Patch, libgfortran] PR59771, PR59774, and PR59836 Bugs in FORMATs Fw.0, Gw.0, and Gw.d

The attached patch fixes these bugs and adds the tests. See the PRs for =
the rationale of the changes.
Regression tested on x86_64-apple-darwin13 and powerpc-apple-darwin9.
OK for trunk, 4.8.3, and 4.7.4 (after testing)?
