C++ PATCH for c++/61959 (POINTER_PLUS_EXPR in CONSTRUCTOR)

It seems that an empty base initializer can show up with an index of 
POINTER_PLUS_EXPR as well.

Tested x86_64-pc-linux-gnu, applying to trunk, 4.8, 4.9.
