[gfortran, documentation] Sketch documentation of gfc_code

Hi Brooks,

here's a first shot at documenting the gfc_code structure.  I also added 
a bit of markup for the index, and changed the filename in the 
@setfilename directive.  It would be great if one could include pictures 
that show how the data structures are linked, but I don't think we
want to blow up gfortran's size that much.
