[incremental] Patch: FYI: initial PCH fix

I'm checking this in on the incremental-compiler branch.

This makes most of the PCH tests pass.  I didn't know it, but when I
split c_write_global_declarations, I needed to preserve the pch_file
early return.

The remaining PCH failures seem to be related to dwarf output.
