[patch] [4.1] Fix PR20256, PR25211 and PR26435 linear transform

Hi,

Here is a backport of the patches that we have in mainline for correcting
all these bugs.  The following patch was bootstrapped and tested on amd64
on the 4.1 branch.  Okay for 4.1 branch?
