[C++] Fix 23044

[Nathan Sidwell]

I've installed this patch on mainline & 4.0.  I had thought we couldn't get a 
SCOPE_REF at that point, but we will when tsubsting when 
processing_template_decl is non-zero.

booted & tested on i686-pc-linux-gnu
