[patch] Remove DBX_OUTPUT_STANDARD_TYPES.

Hi,

Attached is a patch to remove DBX_OUTPUT_STANDARD_TYPES as it is
mentioned only in tm.texi.

Bootstrapped on i686-pc-linux-gnu.  OK to apply?
