[Janitor] Cleanup sdbout.c

This patch cleans up sdbout.c:
- Removes #if 0'ed code
- Removes unused macro RMS_QUICK_HACK_1
- Converts to ISO C90 prototypes

Bootstrapping now on i686-linux-gnu and building a crosscompiler for
arm.

Ok to commit if these pass?

	(sdbout_one_type): Remove #if 0'ed code.
	(sdbout_init): Remove RMS_QUICK_HACK_1 code.
	Remove PARAMS, use ISO C90 prototypes for all functions.
