[patch] Use bit scanning and counting builtins for std::bitset

That's okay, we have a policy of not posting regenerated files in a patch.
(There's no point; they can be regenerated by everyone reading the list.)

Here's what I'm checking in.  Regression tested on i686/linux with no
problems.  In addition to Falk's changes, I did some minor reformatting.
The few occurances of the CHAR_BIT macro are replaced by the builtin
__CHAR_BIT__.  And the bits-per-word define now uses the "canonical"
definition found in <limits>.
