Fixed thusly.  See bugzilla for commentary from all the pricipals.

This will allow certain near-term cleanups, with the eventual goal of
removing the _GTHREADS bits that have moved into libstdc++ sources and
perhaps removing bits/stl_threads.h (which may or may not be possible,
but will now be attempted.)
