C++ misc patch cp-tree.h

All,

This is a long and very boring patch the changelog look longer to
do  than the patch itself.

The attached patch tidies up cp-tree.h w.r.t whitespace, long lines
and use of parenthesis in macros.

Bootstrapped x86-pc-linux-gnu + with and without enable-checking
+ all languages no regressions.
