This is what I found in the earlier thread:

Hmmm, I've got a hardcopy of rev 0, and rev 2 is online at

Appendices C and D in rev 2 have it wrong, but by omitting vC,
not by missorting.  All the other descriptions match.  Is this
a bug specific to rev 1 then?  Interesting.
