including insn-flags.h

This patch moves the inclusion of insn-flags.h into config.h right
after insn-codes.h.
Part of my ia64 eh change to use a configure-time flag instead of a
run-time flag to enable sjlj exceptions requires that insn-flags.h
be included before except.h.
It was mildly tedious rearranging all of the includes. Moreover, if
a file failed to include insn-flags.h before except.h, that file would
silently get a different definition of USING_SJLJ_EXCEPTIONS. Worse,
the changed definition would only happen on some hosts.
Since most files already needed to include insn-flags.h, I thought
the simplest solution was to just include it universally at a known
position.
Bootstrap and check on alphaev6-linux.

final.c, function.c, optabs.c, recog.c, reload1.c, stmt.c,
c4x/c4x.c, i960/i960.c, mips/mips.c:
Don't include insn-codes.h or insn-flags.h.
* genemit.c, genopinit.c, genoutput.c: Don't include insn-codes.h
or insn-flags.h in the generated code.
* genflags.c (gen_proto): Use "struct rtx_def *" instead of "rtx".
(main): Forward declare struct rtx_def.
