Patch to make manuals work with makeinfo --html

Each node is a separate HTML file with its name derived from the node
name.

Further notes:

* Links from one manual to another don't seem to work properly; it should
link to file names of this form but still links instead to the old form
you describe.  (The current texi2html setup doesn't make links to other 
manuals at all, only internal links within manuals.)

* Texinfo 4.0d seems broken (not putting most of the HTML files in the
right directory), but 4.0b (the version on gcc.gnu.org) and 4.0c work OK.
