casesi with HImode index

I third this motion :-) - my patch was just a minimal change, it is good
to see that a more general solution (casesi/casehi/caseqi) exists.
If it will be implemented soon, my patch is not needed (otherwise, I can
send a documentation patch in a few days - thanks for reminding me).
