cosmetic change to insn-extract.c

This patch changes genextract such that insn-extract.c has the insn
names in comments next to each case label. It makes it much easier to
match extract patterns to the md file.

(get_insn_name): No longer a stub.
(main): Call record_insn_name for each insn. After each label
written, print the insn name in a comment/
