Why are dwarf2 symbols getting assigned hidden visibility?

Hi Jonathan,
I already patch ginclude/stddef.h in my parallel repository so I won't 
need to wait for this to get submitted to test it out.  I'll just patch 
my version directly.

I hope these ansi macro changes are the solution.  I've wasted days 
trying to figure this out already!
