fstream and ofstream typedefs

I think it is not required to be defined right in there but rather
that when you #include <fstream> that it is required to be pulled in
and to be defined after the #include line.

When you include fstream, fstream includes istream, which in turn
includes iosfwd, where you'd find:

  typedef basic_ofstream<char>          ofstream;       ///< @isiosfwd
