it seems like there were requests for corrections which Diego said he'd address
in follow-up patches, but I see no evidence that the requested changes were
ever committed.

I think it would be good if someone actually familiar with LTO internals
reviewed the existing internals documentation and corrected it to agree with
the current implementation.  Likewise I don't really know if the existing user
documentation completely reflects current reality, although I think the
concerns about lack of a usage overview and poor integration into the manual
that were mentioned in the issue description have been addressed.
