Updated preliminary patch

This updated patch resolves the issues identified so far.  Please continue
testing. I have regression tested and the testsuite case gfortran.dg/pr20755.f
fails because with the patch I am ignoring the scale factor. Perhaps I have
been too aggressive with that. Regardless, please continue testing while I
study the scale factor question a bit more.
