[Bug libstdc++/37391] examples not accessible online (non existing URLs)

All the test and example links in pb_ds that point to source files are broken.
These include links linked to from these pages:

libstdc++-v3/doc/html/ext/pb_ds/assoc_examples.html
libstdc++-v3/doc/html/ext/pb_ds/pq_examples.html
libstdc++-v3/doc/html/ext/pb_ds/assoc_tests.html
libstdc++-v3/doc/html/ext/pb_ds/pq_tests.html

These links are fine when browsed in the source tree, but the on-line version
does  not include the sources that these links point to, so thus the broken
links.

The way to fix this would be to include the example files inline in the
documentation, or link to cvsweb or other web versions of the pages.
