[f2003] ACCESS='STREAM' io support

FX,

A search of the list shows me that you had intended to implement this 
functionality.  Are you still doing so?  I don't want to interfere or double up 
on anyone.  If so, let me know, and I will retreat.

I do have the frontend accepting ACCESS='stream' and passing it to the library. 
  I see we need to deal with running out of bits for INQUIRE so I will go ahead 
with what Jakub proposed on this in:

Also I am looking for some sample code for F2003 using this feature.  I am 
googling for it now, but if anyone has some examples, that would be appreciated.
