And thank you for the quick review!  I've attached the version of the 
patch that commited to 4.2; that was also tested with "make info" and 
"make pdf" before committing.

For the record, at this point the documentation differences from 4.2 to 
trunk consist entirely of additions to the "Fortran 2003" section.
