Merging 4.1 changes back to 4.0

Many more.  Almost at 4.1 ChangeLog entries that contain fortran/PR #
that is missing in 4.0 can be committed.  Jakub's entry patch should
definitely be committed.  Also, the texi files should be synchronized.
