After applying the patch to the 4.0 branch, it succeeded with fuzz = 1.

There are some patches applied to 4.1 that have not been transferred to the 4.0 
branch yet.  So when we apply this latest to the 4.0 branch we may have to do it 
manually or with some minor tweaks for it to play.  I think this is what you are 
seeing.

I have attached an UNTESTED diff to 4.0 branch if you are in a hurry and want to 
test.  I will start testing this tonight.
