[v3] doc hacking


This updates the libstdc++ manual to trunk behavior with respect to
documentation generation. This has changed for 4.6, largely based on
the feedback provided on this ML and the gcc wiki page on libstc++ docs:

These edits intend to fold in any relevant observations so noted on
this wiki page, and expand the current information with all the issues
that people have pointed out.

Tis still not perfect but I think it's better, and more importantly
accurate. Follow up edits are most welcome.

ps. I realize that the absolute paths for getting the build diagram are
a hack (re build_hacking.xml change in this patch). Before, the images
were unceremoniously stripped from HTML and PDF output. Now they are
around, but only on my machine. Help!
