[libstdc++ PATCH] Deprecate basic_string::insert(iterator)

This little patch deprecates the GNU extension that inserts a default-constructed character into a string if no character value is provided. The same type of extension was deprecated in 3.3 for vector et al, but this one was apparently missed.

Tested on powerpc-apple-darwin; no regressions. Okay?

Doug
