[libstdc++] Slight doxygen reworking, more markup



Our first guest doxygenator has already sent in markups for two large
files; that patch is included here.  Also, this renames mainpage.doxy to
mainpage.html and uses it directly; Doxygen's handling of raw HTML files
leaves a lot to be desired.  Finally, all the library-related "implementation
defined" behavior is documented, like the standard says we should.
