Pseudo-unreviewed patches

There are three patches that I've had in my local tree for a while. I'd
want to either check them in or throw them out before starting work on the
"solaris long long" problem. In random order:
[1] fstream should check the result of explicit close()
http://gcc.gnu.org/ml/libstdc++/2001-10/msg00113.html
Nathan asked a question, I answered, no more responses.

Nathan talked some about the idea, but I never got any followup to this:
http://gcc.gnu.org/ml/libstdc++/2001-09/msg00023.html
It's the "ignoring openmode app" fix.

Say the word and it's in. :-)
Actually, my doxygen markups are about to overlap the c-c changes, so I'd
really like to get this in before doing much more, uh, doxygenning.
