Fixincludes cleanup + system headers

While you're at it, how about collecting sets of published system headers
and sort them by platform/os/os-version? We can put them into a
CVS hierarchy and use them for regression testing of fixincludes.
It might be another way of figuring out which fixes can be obsoleted. :-)
I'm certain there's dead code in there....
