[Patch, Quadmath] Update to headers (PR 46817)


I intent to commit the patch as obvious in about a day.

When I added the copyright header to the files lacking them (written by FX and myself for GCC's libquadmath), I seemingly missed to change "libiberty" to "libquadmath" (cf. PR 46817). Additionally, I take the opportunity to change the "//" comments to "/* */" comments and to add "2011" to the copyright year.
