The obvious reason was of course that I overlooked that *I*
could change texmf.cnf (at least temporarily for purpose of
testing this patch, even though that solution wouldn't be
suitable for mass consumption). Doh!

Thank you. Committed, though I changed the part of the comment
that said "work around memory limits in older tetex
distributions" to "work around memory limits for the default
configuration in older tetex distributions".
I also committed this to the 3.4 branch, after checking that it
solved the problem there too. The problem does not appear using
tetex-1.0.6 on my "Mon Feb 23 18:58:32 GMT 2004" checkout of the
3.3 branch, where md.texi was up-to-date.
